At Donato Dental, we have been using CEREC Technology to create natural-looking porcelain restorations since 2004.
Donato-Cerec1The old amalgam restoration was removed and a bonded core build-up was placed. Tooth was then prepared for a porcelain crown utilizing a microscope.   Photos were taken through a camera attached to the microscope.   What fun to be able to share information and a case or two with anyone interested in learning!Magnified view of the crown preparation. The margin of the preparation was just under one millimeter in depth.An optical impression of the crown preparation was obtained and a crown restoration was custom-designed on a computer-generated 3D model.The crown-design data from the computer is then sent to the milling machine which will fabricate the porcelain restoration from a solid block of porcelain in about 15-20 minutes. Step 1: old (leaking) crown removed from subject toothThe adjacent tooth with the silver filling has a ledge (overhang) of filling material below the gum line. Overhangs accumulate plaque and makes it difficult for patients to floss without getting the floss hung-up. While I am in there working, I will address issues like this whenever I have access. Removing the old crown therefore provided access and an opportunity to smooth things up!Amalgam overhang being smoothed which will help make flossing easier for this patient.Amalgam overhang of the other adjacent tooth being smoothed here too.   Ok, so back to the subject tooth…there is an existing composite build-up which appears to be separated from the natural tooth structure (looks like a fracture). This could mean that what lies underneath is…..(next slide please)…Decay!!! Lots and lots of soft cheesy decay!Decay is all cleaned up and the little remaining tooth structure is now ready for a fresh new Core Build-Up. A core build-up will help retain the crown in this case.A bonded composite material is being used for the core build-up. The material is mixed and put into a syringe with a narrow-gauge needle and applied with the aid of a surgical microscope.  (Photos are obtained from a camera attached to the microscope which makes it very easy for my assistant to snap away while I’m working. The crown is designed on a computer and a milling machine fabricates the porcelain crown to the exact design and specifications.Final CEREC Porcelain Crown complete and another tooth saved from extraction!

One of the great things about using Dental Operating Microscopes (with digital cameras attached to them), is that my trigger-happy assistant can snap away while I’m working! Showing patients what I see (before, during and after treatment) is a big plus over trying to explain what I saw and did through words alone.
